Affirm Life: Claimant's Guide

Kindly submit the following documents in support of your claim:
- Claimant’s Statement – This form is to be completed by beneficiary (ies) of the policy
- Attending Physician Statement (This form is to be completed by the Physician at attended to the deceased at the time of death)
- Supplemental Physician’s Statement – ONLY if requested by Claims Examiner (To be completed by the personal Physician with history of deceased)
- Police Report if death was due to an accident
- Grant of Probate or Letters of Administration (if the beneficiary is the Estate of the deceased)
- Lost Policy Declaration form if the Original Policy Contract cannot be located
- Original Policy Contract
- Death Certificate (Certified by a ScotiaLife Officer, Bank Officer or Notary Public)
- Proof of age of the deceased – E.g. National ID, Passport or Drivers Permit (Certified by a ScotiaLife Officer, Bank Officer or Notary Public)
- Valid ID of the Claimant / Beneficiary - E.g. National ID, Passport or Drivers Permit (Certified by a ScotiaLife Officer, Bank Officer or Notary Public)
- The Death Benefit shall be payable within 5-7 business days after written proof of claim satisfactory to the Company has been received.
